Who we are
Queens Pharmacy operates four community pharmacies in Kitchener, Ontario. As a health information custodian under Ontario's Personal Health Information Protection Act (PHIPA), we are responsible for the personal health information in our care.
What we collect on this website
When you use our online forms we collect only what's needed to act on your request: your name, date of birth, phone number, the store you chose, and the details of the refill, transfer, prescription image, appointment or message you send. We do not collect your health card number online.
How we use it
To fill or transfer your prescription, book your appointment, or answer your question, and to keep the records the Ontario College of Pharmacists requires us to keep. We do not sell or share your information for marketing.
Where it's stored
Requests made on this site are encrypted in transit and stored on servers located in Canada. Only staff at the store you selected, and the pharmacy owner, can view them. Completed requests are removed from the website system on a regular schedule once they've been actioned in the pharmacy's dispensing software.
Cookies and analytics
The public pages of this site do not use tracking cookies. The staff sign-in area uses a session cookie strictly to keep staff logged in.
Your rights
You can ask to see the personal health information we hold about you, request a correction, or ask how it's been used. You can also make a complaint to us, or to the Information and Privacy Commissioner of Ontario at ipc.on.ca.
